조대술을 이용한 비교적 큰 함치성 낭종내 전위된 치아의 맹출유도에 관한 치료증례

Abstract
서론
함치성 낭종은 치관이 형성된 후 성상망의 변성으로 인하여 치관과 환원 법랑질 상피사이
에 유동물질이 축적되어 나타난다. 이들은 악골의 어느곳에서나 발생하나 하악골에 빈발하
며 미맹출치아의 치관이나 과잉치, 매복치아와 관련되어 나타나지만 특히 하악 제3대구치와
상악 견치부위에 호발한다.
연령별로는 10대 또는 20대에서 발생율이 높고 성별에 따른 발생빈도는 별 차이가 없으나
남자에서 약간 높은 빈도를 나타낸다.
이러한 낭종을 불안전하게 적출되거나 적절히 치료되지 못하는 경우는 높은 재발율을 보
이며, 벽제성 법랑아세포종, 상피암종등으로 이행될 수 있으므로 그 치료에 있어 특히 주의
를 기울여야 한다.
낭종의 치료 방법은 조대술과 적출술 그리고 조대술을 이용해 낭종의 크기를 줄인 후 적
출해내는 caldron의 2단계 수술법이 있다.
이중 조대술은 낭종벽을 deroofing시키고 구강점막과 연결시켜 주는 술식으고 골 내압이
감소함에 따라 결손부 내 골 재생이 일어나게하며 낭종벽은 정상적인 점막으로 변하게 된
다. 또한 인접 주요 구조물을 보존학 수 있으며 oronasal 또는 oroantral fistula를 형성하지
않으며 적출술시 발생할 수 있는 인접 정상치의 실활이나 혈류 공급의 방해를 방지할 수 있
다. 특히 이 방법은 소아에서 낭종에 이환된 치아를 불필요한 손상없이 정상 맹출시킬 수
있는 큰 장점을 가지고 있다.
함치성 낭종에 포함된 미맹출 치아의 치료시, 작은 낭종과 관련되어 변위가 적은 치아에
서는 치아의 자연 맹출에 의한 보존이 비교적 용이하나, 큰 함치성 낭종과 관련된 미맹출
치아의 위치이상이 현저한 경우에는, 치아맹출에 대한 예후를 판단하기 힘든 바 발치되는
경향이 강하였다.
이에 저자는 형성과정중의 치아가 함치성낭종에 의해 전이가 현저할때 교정적인 견인없이
조대술과 보정 장치만 이용하여, 자연맹출을 유도하여 양호한 결과를 얻었기에 이에 보고하
는 바이다.

The dentigerous cyst originates through alteration of the reduced enamel epithelium
after amelogenesis is completed, with accumulation of fluld between the layers of the
enamel epithelium or between this epithelium and the tooth crown. It is always
associated with unerupted teeth. there are two methods of treatment which are generally
accepted. One is the enucleation of the cystic sac in its entirety, the other is the Partch
operation, or the marsupialization, by which the cyst is uncovered or 『deroofed』 and
the cystic lining made continuous with the oral cavity or surrounding structures.
We have 3 cases involving management of impacted & displaced teeth after
marsupialization of a dentigerous cyst. We would like to summarize the results of the
report as follows:
The teeth which had been displaced near the bottom of mandible and dislocated
distally or mesially by the pressure of the cyst on which had been displaced and
inclined abnormally by the pressure of the cyst, have a tendency to erupt rapidly into
their normal position soon after marsupialization, even without the application of
orthodontic measures.
